Method 2: EMAIL / FAX ORDER

Fax orders to (905) 677-0023 or e-mail to info@anogen.ca.
You may use your personal order form, or you may complete the attached Printer-friendly Order Form, print out a copy and fax the order to Anogen.  Required Information including:

1. VISA or Master Card information (number, expiration date, card holder name) or Purchase order number for returning customers;
2. Shipping and billing addresses;
3. Catalog number, description, and clone number;
4. Quantity, size and price;
5. Courier name and account number (if applicable);
6. Expiry Date Requirements (if applicable);
7. Delivery Requirements (if applicable).

Once you have placed an order, you will receive an order confirmation. If you do not receive a confirmation of your order within 24 hours please contact us at: 905-677-9221 or Toll Free at 1-877-755-8324 x 233.

Credit cards will be charged in American Currency.
We require pre-payment via bank wire transfer or credit reference for the first order placed with Anogen in order to establish credit for new customers. Bank information will be provided upon receipt of your purchase order. Returning customers may pre-pay by credit card, or request a Net 30 Day invoice.
Shipping charges are extra. A handling charge of $50.00 USD is applicable on orders under $200.00 USD. You will receive an order confirmation via e-mail indicating your total shipment cost. Due to variations in Foreign and Domestic Shipping Regulations and Policies, we are unable to provide static Shipping charges.
All orders are shipped according to their storage requirements and are shipped via Federal Express Next Day Service (service does not guarantee next day delivery).
All products are shipped with a minimum shelf-life of 6 months. If you require an extended shelf-life, please specify.

FDA / USDA:

All of our products are labeled in accordance with USDA.  Please ensure that all required import information is available for dispense at the USDA's request. Select from the attachment to view a sample letter that Anogen will supply to the USDA:

6. Polyclonal (not including poultry or livestock) USDA Letter
USDA import permits are not required pursuant to Guidelines for Importation #1121 (Effective October, 1998) for products not intended to be used to diagnose disease or illness of humans and animals. Monoclonal antibodies, ELISA kits and recombinant proteins do not require additional documentation for import. 
Animal products or animal by-products containing poultry or livestock derivatives (chicken or goat polyclonal antibodies) require an APHIS Import Permit issued by the Veterinary Services, National Center for Import and Export (NCIE). Applications for import permits are located online and must be completed and submitted by the Importer (Customer) electronically or by fax to (301) 734-8226.

Additionally, an annual application for the Permit to Import or Transfer Etiological Agents or Vectors of Human Disease PHS from the CDC can be obtained and will assist the entry of infectious agent products. To apply for this permit (Click here for Adobe file: Form CDC 0.753 REV 02/04), contact:

It is the Customer's responsibility to insure all import documents are available for immediate dispense at the request of the USDA. In some circumstances, the USDA may contact the Customer to obtain additional information as to the applications or use of the product. The Customer must ensure that all communications with the USDA are prompt and thorough. If the USDA refuses clearance of the product based on the Customer's insufficient cooperation or documentation, Anogen will invoice the Customer for return shipping costs and the cost of the product if it fails to pass quality testing due to improper storage or handling while in the possession of the USDA.
